Telehealth Express Consent and Waiver of Liability:

 

This Telehealth Express Consent and Waiver of Liability shall be between You, (the client/reader/viewer/user/other of this site and of services) and the Coach, (Francesca Esposito, CARC, or Phoenix Coaching Services, LLC) By using this website and/or by engaging the services now and in future, of the coach, Francesca Esposito, CARC, or Phoenix Coaching Services, LLC, you hereby expressly consent and agree to the following: a) To engage in telephone and/or teletherapy counseling services. Which shall be called "teletherapy" in this document. b) I understand that "teletherapy" includes the practice of health care delivery, consultation, treatment, transfer of medical data, and education using interactive live audio, video, or data communications. I understand that teletherapy involves the communication of my medical/mental information, both orally and visually, to the health care coach and/or practitioner located in the state of New
York. c) I understand that I have the following rights and understanding with respect to teletherapy:
1. I have the right to withhold or withdraw consent at any time without affecting my right to future care or treatment
nor risking the loss or withdrawal of any program benefits to which I would otherwise be entitled.
2. The laws that protect the confidentiality of my medical information also apply to teletherapy. As such, I will need to attest that I am in a private, non-public, secure place, and alone for each of my teletherapy sessions.
3. I understand that the information disclosed by me during the course of my therapy is generally confidential. I understand there is a risk associated with the use of technology and I accept these unforeseen risks. I will not hold the coach, Francesca Esposito, CARC, or Phoenix Coaching Services, LLC, liable for or responsible in the event of any breach to or cyber security risk or threat of my personal and/or confidential information.

 

Insurance, Payments, and Fees:

We do not accept insurance. Clients shall pay for all services out of pocket. By using our services you understand and agree to these conditions. All *sessions must be paid for in advance of services. Payments are accepted via Zelle, PayPal, and Venmo.  Any session will be canceled with or without notice at our discretion, for lack of payment, or for any other reason, if not received within 24 hours of the start of the session. *If a session is a complimentary 15-minute session, then payment is not due until within 24 hours prior to the first full session and each session in advance thereof.

 

Cancellation, Missed Session, Lateness, No-show and/or Refund Policy:

We do not offer any product or service credit or refund for a No-Show, excessive lateness, or a missed session. In event of your cancellation, please do so prior to 24 hours of the session start- (1) day.
